Petition to Receive an Incomplete ('I' Grade)

Submitting this petition does not guarantee approval.  In order to be considered for an Incomplete ("I" Grade) the student must submit this form and it must be approved by the instructor and the Dean of the division by the following deadline: 

  • Traditional and Online UnderGrad students - before the start of finals.
  • ADC(SPS) and Grad students - one week prior to the end of the course.

Preparation for completing this form:

* Students must be in good academic standing in a course when the incomplete is being requested.

* Students must only have a nonsubstantive portion of their coursework left to complete at the time of the request.

  1. Meet with the instructor to agree on the remaining work to be submitted and the due date.
    1. Due date must be no later than 10 weeks from the end of class.
  2. Have the following documentation to be submitted in this form: (Documents must be in PDF form.)
    1. Documentation from a medical professional, insurance claim, police, etc. of the special circumstance. (if, applicable)
    2. Outline from the Instructor of the incomplete work that must be submitted. (mandatory)

Form Processing:

  1. The instructor, the Dean, and the Registrar will be notified and asked to review the petition.
  2. Your Success Coach will email you regarding the approval/denial of your petition.
  3. A $35 processing fee may be charged.
