This form should be completed by students who are placed on financial aid probation because they did not meet the requirements for Satisfactory Academic Progress (SAP) and who wish to request an extension of financial aid eligibility. This appeal should be submitted along with any required documentation detailing the extenuating circumstances that contributed to you not meeting the requirements for SAP.

Medical condition - a physician's or health care provider's statement confirming your medical condition.
Death of a family member - copy of death certificate or link to obituary
Divorce/Separation - court documents
